🔢 STEP 1: Select Your Sponsorship Frequency / Number of Mailings ▼
To keep your custom data securely synced with our payment system, additional mailings are added onto the base price. Selecting 2 Mailings adds $3,500 (the cost of one additional mailing) to the base price of one mailing. Selecting 3 Mailings adds $7,000 (the cost of 2 additional mailings) to the base price of one mailing. Your final invoice total will reflect our standard rate card.

📋 STEP 2: Select Your Sponsorship Type ▼

• Exclusive Single Sponsor ($3,500.00): Includes a full page featuring the sponsor plus a full page sponsored contextual content page from Cardio Renal Foundation, and a logo plus QR code feature on the mailing envelope for 100% Share of Voice to 1,000 affluent residents in your community. A permanent digital placement on the Community page of the cardiorenalfoundation.org website will also be included with our gratitude. This package also includes one-year unrestricted print and digital licensing rights to feature our charity logo across your corporate marketing campaigns. Total sponsorship package value: $10,500+
🔒 STEP 3: Select Your Professional Field/Industry ▼
Your sponsorship is category exclusive. Select your professional sector from the drop down menu. Once you lock in your mailing date(s), your competitors are completely locked out of that specific campaign run.
📍 STEP 4: Confirm Your Region ▼
Enter the local region that you wish to target. Include neighborhood(s), city, zip codes(s).
🖼️ STEP 5: Upload Your Logo Assets ▼
Vector files (EPS/SVG) or high-resolution images are preferred for best print reproduction. Drag and drop your creative asset files directly into our secure upload panel block during your application path. Files with a transparent background are preferred for clean print production.
✍️ STEP 6: Provide Your Custom Mailing Copy ▼
Provide your slogan, website, email address, phone, and/or mailing address that you want printed on the materials. Also include a two-to-three sentence description that you want consumers to read.
📅 STEP 7: Lock-In Your Calendar Dates ▼
Let us know what date(s) you would like to secure for your mailing. Remember, mailings are delivered to the USPS on Fridays. When providing your desired mailing date(s), please allow ten days of lead time.
🏷️ STEP 8: Apply Your Negotiated Discount Code (If Applicable) ▼
If a member of the Cardio Renal Foundation team has provided you with a negotiated discount over the phone, please type that specific code into the Discount Code box inside the form. To ensure the deduction is subtracted from your final bill, you must also re-enter that exact code word in the native discount field on the final checkout screen before processing your payment.
💳 STEP 9: Check Out ▼
Once all your custom form details, logo uploads, and mailing dates are filled out, click the Add to Cart button. Verify your package total balances correctly ($3,500 for 1 mailing, $7,000 for 2, or $10,500 for 3), then proceed to the secure billing section to enter your credit card or express wallet details to finalize your exclusive territory reservation. Be sure to re-enter the Discount code that was provided to you on the final checkout screen before processing your payment. Because this program carries a $0 Fair Market Value, you will receive your official 100% tax-deductible charitable contribution receipt via email automatically when your transaction processes.
Please note, your sponsorship is processed in our system under a standard physical verification profile. As a result, your checkout invoice may include the terminology "Preparing items for shipping." Please be advised that this is simply our standard technical framework - absolutely nothing will physically ship to you. A member of our creative team will follow up as we prepare to deploy the direct mail campaign.
